I understand that mixed martial arts training and related event activities may involve strenuous physical exercise, partner drills, controlled contact, falls, and other activities that carry an inherent risk of injury. I voluntarily choose to participate with full knowledge of these risks.
I confirm that the medical, emergency contact, and personal information I have provided is accurate and complete to the best of my knowledge. I agree to notify EazyFights Academy before participating if my health condition changes or if I become unable to participate safely.
I understand that photographs, videos, audio recordings, and other media may be captured during training sessions, competitions, seminars, and other event-related activities. I grant EazyFights Academy, its organizers, partners, and authorized representatives permission to use, reproduce, edit, publish, and share such media featuring me (or my child, where applicable) for promotional, educational, marketing, social media, website, and other lawful business purposes without further notice or compensation, unless otherwise required by applicable law.
By accepting this waiver, I voluntarily agree to participate in the event and release EazyFights Academy, its coaches, staff, organizers, partners, sponsors, and affiliated representatives from liability for injuries, damages, or losses arising from ordinary participation in training or event activities.
